Photo Usage Consent

I grant permission for my photos to be used by Slay Squad ("TSS") for promotional purposes. If, at any point, I no longer wish for my photos to be used, I must contact [email protected] to request removal.

Intellectual Property & Confidentiality

I agree not to record, reproduce, distribute, or resell any videos or information from this program.

Email Communication

I understand that I will be added to the Slay Squad email list for program-related communication. I may unsubscribe at any time.

Assumption of Risk

I acknowledge that participation in exercises, services, and the use of equipment involves inherent risks, including but not limited to bodily injury, dehydration, sun exposure, slips, falls, and even death. I knowingly assume all risks, including financial losses resulting from any injury, whether caused by my own actions, the negligence of TSS, or third parties.

I further acknowledge that unexpected dangers may arise and that I voluntarily assume all risks involved. Additionally, I waive my right to a jury trial as outlined in Article 18, Section 5 of the Arizona Constitution.

Release of Claims

I, on behalf of myself, my heirs, administrators, and representatives, fully release and discharge TSS, its officers, directors, employees, contractors, and affiliates from any and all claims, liabilities, damages, or expenses related to my participation in the exercises, services, or use of equipment—whether arising from negligence or otherwise.

Covenant Not to Sue

I agree not to initiate any legal action against TSS, its officers, directors, employees, or affiliates for any injuries, including but not limited to personal injury, property damage, or death, arising directly or indirectly from my participation. If I violate this agreement, I am responsible for covering all reasonable attorney fees and legal costs incurred by TSS.

Release of Liability – Pregnancy

I acknowledge that starting a new training or nutrition program while pregnant should only be done with approval from my healthcare provider. By checking this box, I confirm that I have reviewed my workout plan with my provider and received approval before beginning.

I understand that pregnancy poses additional risks, including but not limited to injury, complications, or loss of life for myself or my baby. I knowingly assume all risks and release TSS from any liability associated with my participation. I agree to exercise at my own risk and recognize that TSS is not responsible for any injury or loss.

I further waive my right to a jury trial and acknowledge that I assume full responsibility for any consequences resulting from my participation.

By checking this box, I accept the terms and release TSS from any claims or damages related to my participation in this program.
